- Information you submit through signup and inquiry forms: your name, email, phone number, and the details you choose to share about your clinic, practice, or membership interest.
- Basic usage data (pages visited, approximate region, device type) when analytics are enabled, used only to understand how the site is used.
How we use it
- To respond to your inquiry and set up your membership or partnership.
- To operate, secure, and improve the site.
- To send you communications you have asked for. You can opt out at any time.
What we do not do
- We do not sell your personal information.
- We do not share your information with third parties except service providers who process it on our behalf (hosting, email delivery, analytics) under contractual confidentiality obligations.
Health information
The marketing site does not ask for medical history. Clinical intake happens inside the member experience after signup, where it is collected under the practice’s clinical policies and applicable law, and is used solely for your care.
Retention and access
We keep inquiry information for as long as needed to serve you. You may request access to, correction of, or deletion of your information by contacting the practice, subject to record-keeping obligations that apply to medical practices.
