Telehealth Informed Consent

Telehealth means receiving care from a licensed clinician through electronic communication: video visits, asynchronous review of your intake and records, and secure messaging. Before receiving care through MYST Medical PLLC, you should understand the following.
How telehealth works at MYST
- Your intake and health history are reviewed by a licensed provider.
- Some care is asynchronous: the provider may review and decide on your request without a live visit when clinically appropriate.
- You may be asked to complete labs or an in-person evaluation when the provider determines it is needed.
Benefits and risks
Telehealth improves access and convenience. Its limitations include the absence of hands-on examination, dependence on the accuracy of the information you provide, and the possibility of technical failures. Care may be delayed or redirected to in-person treatment when telehealth is not appropriate.
Your rights
- You may withhold or withdraw consent to telehealth at any time without losing access to alternative care options.
- You will be told when a therapy is declined and why, and you may request review.
- Your records are maintained under the practice’s clinical policies and applicable law.
Emergencies
Telehealth is not for emergencies. If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency room.
